I track lots and lots of threads, and many of those threads complain about a search result. Many of those complain about inappropriate images showing up in Google Images. Google launched a report offensive images about two years ago, and they changed it slightly later. The issue is, it does not work. I cannot tell you how many times I have seen images reported in the forums, I then personally go and report them using the "Report Offensive Images" link in Google Images and I wait for nothing to happen.
I then ultimately post the example issue here and it gets taken care of after a Googler reads it on this blog and is embarrassed. But honestly, I would post a new example each day if I wanted to simply report embarrassing, insulting or incredibly offensive search results in Google Images. Now, some Google won't remove but some they will - such as porn and nasty images.
Last Friday, I saw a report in Google Web Search Help of searching for [headache] in Google Images returns a picture of a face completely destroyed. The image is extremely upsetting and bloddy. So I warn you, do not search for it if you are at all squeamish.
So on Friday, I reported the image by searching for the keyword, clicking on "Report Offensive Image" under SafeSearch and reported the image. So did others, that same day. I waited and waited and even today, the image is still there.

Honestly, I have never been able to report an image this way and have it removed without writing about it over here. So does this really work? Does Google man those reports? What is the issue?
